Preparing for your first appointment at Rock Creek Neurosurgery and Spine
We are pleased that you have chosen Rock Creek Neurosurgery and Spine for your neurosurgical care. We strive to provide the highest quality service at all times. After you have schedule your appointment an email will be sent to you with a link to the patient portal. The patient portal is a secure method which will allow you to complete your personal patient information prior to your visit. The patient portal will allow completion of patient forms, later updates, pay your bills and communicate securely with our practice. It will also let you make new appointments or reschedule appointments.
If preferred, after you are scheduled for an appointment, you will receive a packet by mail with items to complete for your first appointment. If you have not received a packet, call (801) 609-9310 or contact us.

- Complete your patient demographic information prior to your first visit This patient packet can be accessed through and filled out through the patient portal. Alternatively, this packet of information can be mailed to you. Having completed your paperwork will expedite your visit with Rock Creek Neurosurgery. For the greatest convenience, we encourage you to use the patient portal to complete these forms.
Please Bring to Your Appointment
- Insurance cards
- Referral or authorization forms.
- Name and address of your family doctor and other referring physician.
- Your completed patient information paperwork unless completed through the patient portal.
- Copies of Images (MRI, CT, XRays)- Please note that our staff will personally review the images in addition to reviewing the radiology reports. Please bring the CD copy of images in addition to the report. If images have been done at an HCA facility we will have a access to those images.
- Any other written reports such as EMG, EEG, NCV, outside surgical consultations.
- Workers’ compensation claim allowance letter, if applicable.
- Payment method for copay, due at time of service, or other services not covered by insurance, including copayment, coinsurance and deductibles.
