Referrals

We want to make it simple for you to refer patients to our care. There are multiple ways to submit a referral to Rock Creek Neurosurgery and Spine.

1. Secure Online Form: The secure online forms can be accessed in one of two ways:
   • Patient Portal: This service allows for patients to personally complete the referral information necessary for an evaluation, and schedule an appointment themselves. Furthermore, this facilitates a patient’s visit by allowing them to complete demographic and other information prior to their visit. We encourage you to direct your patients to this site located on our webpage at rockcreekneurosurgery.com. Physician staff will need to fax the referral form
2. Fax: To make a referral by fax, please download and print the neurosurgical consultation form here. The appointment fax number is (801) 465-0901. Then fax the completed consultation form, pertinent patient medical records including face sheet, radiology reports and any EMG/NCV results.
3. Direct Phone Call: To request a neurosurgery consultation, your staff may call our clinic office directly at (801) 609-9310. We prefer that you reserve direct phone calls for appointments for those consultations that are of an urgent nature only.

Urgent Consultations

If you have a patient with an urgent or emergent neurosurgical condition, we are committed to be a resource to you as the primary care provider and as a guiding specialist for your patient and their care. Typical urgent problems may include acute or rapidly progressive neurologic deficits such as weakness or myelopathy, infections, loss of bowel or bladder suggestive of Cauda Equina syndrome, intractable pain or as otherwise determined by the patient’s care providers. If a patient is determined to have an urgent or emergent neurosurgical condition, please make the referral in one of the methods shown above. If such consultation is needed outside of regular hours, please contact us.
